Adaptive Equipment Requests

                 Adaptive equipment, therapy equipment, communication devices, you name it, are things many people with disabilities need.  Chel’s Hope Foundation seeks to provide assistance to disabled persons who may require and cannot afford such equipment for their daily living needs.  If you are a person with a disability and would like to request assistance in acquiring adaptive equipment, please submit the following:

v     A brief written statement telling us, in your own words, about yourself and your daily life.

v     Summary of a person’s disability, written by a doctor on letterhead stationary.

v     Document must be signed by the professional, identify his/her credentials, and include the address of the office.

v     The summary should describe the person requiring the equipment as a child, teen, young adult or adult.

v     Summary should name the disability using professional diagnostic language and a discussion that a lay person can accurately verify the disability exists.

v     Type of equipment being requested and reasons the equipment is required.

v     It should explain how not having the equipment interferes with daily living.

v     Proof of financial assistance must accompany the request for adaptive equipment, latest tax return and a financial statement.
