SCHOLARSHIP DEADLINE SCHOLARSHIP DUE: Four (4) months prior to tuition due date *Scholarship Form Physician's Certification Form Financial Statement* The scholarship program is a long-term commitment to brain-injured students entering and attending college, technical trade school, or any institution of higher learning. Trade schools include but are not limited to, art, culinary arts, electronic, legal, and interior design, etc. Applicants will be contacted by phone, email or mail regarding their submission. Scholarship application requirements are: § Brain-injury. § 2.5 GPA (minimum) official transcript required. § Two essays 250 words each max (please see application form for subject matter). § One letter of reference submitted with application (see application form for details). § Proof of financial need will be required, most recent tax return and a financial statement. § Physician’ certification of brain-injury and proof of disability will be required. § Submission MUST be mailed.
